Categories open using set template

I can't seem to get this to work. I created a sub type called test. Edited the form how I want. Go into categories and set that category under Incidents to open Test sub type. I create a ticket with said category and it just opens in default template. What am I doing wrong?

Hi sleeper,

In the Category options screen you can select templates to automatically be chosen if the category is selected. Did you also configure a template to match this new sub-type?

Can you post screenshots of the sub-type and its related template, as well as the category configuration screen, and the final result in the Submit/Edit SR?
