we have a user that was unable to access out ticketing portal. upon investigation we have discover they have two user entries in our system. 

has anyone encountered this?
does anyone have suggestions on what happened? 
how to prevent? 

how to remediate? 
 

the AD and Azure details/properties appears to be correct

(email should not have numbers at end of name or have “onmicrosoft” in domain)
