What does the CMDB field do for importing categories?  If we have it on, what does it do when something is set to that category?

The Import Categories section mirrors the page where you configure a category one by one: 

 

If the checkbox for CMDB is checked that means that when configuring and CI on the CMDB the Category you created will be available in the dropdown for your choosing.
