Here’s how you can create a custom "Room Number" field and add it to the Request/Incident Submission Forms in the Self-Service Portal (SSP):

Step 1: Create the Custom Field

1. Navigate to Settings:
   • Go to Settings > Customize > Field customization.
2. Add a New Field:
   • Select the relevant entity(Service Record).
   • Click on New to Add Field.
3. Configure the Field:
   • Set the Field Name to "Room Number".
   • Choose the appropriate Field Type (e.g., Text, Number, etc.).
   • Save the field.

Step 2: Add the Field to the Form

1. Open Form Designer:
   • Go to Settings > Self-Service Portal
2. Edit the Form:
   • Select the Request or Incident Submission Form > Designer Form.
3. Add the Custom Field:
   • In the Available fields column, look for the "Room Number" field and move it into the Visible fields column.
4. Save Changes.

Step 3: Test the Form

If you use Spaces, please refer to the below steps:

   • Drag and drop the "Room Number" field into the desired section of the form.
4. Save Changes:
   • Save the form to apply the changes.

• Open the SSP and submit a test request/incident.
• Verify that the "Room Number" field appears and functions correctly.
