How do I add a place for users to add in the room number, in the user information when they create a incident? I want to edit the template so they can put in the room number they are in that they are requesting help.

Hi
Here’s how you can create a custom "Room Number" field and add it to the Request/Incident Submission Forms in the Self-Service Portal (SSP):
Step 1: Create the Custom Field
- Navigate to Settings:
- Go to Settings > Customize > Field customization.
- Add a New Field:
- Select the relevant entity(Service Record).
- Click on New to Add Field.
- Configure the Field:
- Set the Field Name to "Room Number".
- Choose the appropriate Field Type (e.g., Text, Number, etc.).
- Save the field.
Step 2: Add the Field to the Form
- Open Form Designer:
- Go to Settings > Self-Service Portal
- Edit the Form:
- Select the Request or Incident Submission Form > Designer Form.
- Add the Custom Field:
- In the Available fields column, look for the "Room Number" field and move it into the Visible fields column.
- Save Changes.
Step 3: Test the Form
If you use Spaces, please refer to the below steps:

- Select the form you want to modify (Request or Incident submission form).
- Add the Custom Field:
- Drag and drop the "Room Number" field into the desired section of the form.
- Save Changes:
- Save the form to apply the changes.
- Open the SSP and submit a test request/incident.
- Verify that the "Room Number" field appears and functions correctly.
