Glad you reached out 🙂 the answer has 2 parts:

1. With your license for incident tickets only you should be able to add custom fields here:

Settings> Customize> Fields Customization (Entities)



2. If you are interested also in Change/Request/Problem tickets this is an additional module called ITIL, reach out to our lovely Customer Success team for more information about what it includes and costs:
