Hello: Is there a way to add a block of text on the incident template to provide the end users instructions or additional details when they submit? This is under ADMIN ==> SELF SERVICE PORTAL ==> Incident Submission Form. Thanks

You can create a custom field (under Settings > Customize > Entity > Service Record) and write the code on "Display in Self-Service Portal form" field to display the instructions. Then add that new field to the Incident Submission Form. If you are using a Cloud account, contact your Account Manager to get help.
